We will require the following documents to prove your exposure and validate your claim:
- Certified copy of your ID
- Completed asbestos questionnaire
- Proof of Exposure from a family member who worked on the mines (mine record or correspondence) or
Proof of environmental exposure due to living on or near a mine/mill - Medical reports confirming diagnosis of Mesothelioma
- If you have made an application to either the Asbestos Relief or Kgalagadi Relief Trust – all correspondence (paperwork) should be provided
If deceased, the next of kin may make a claim if the date of death is in the last 3 years. We will require the following documents to prove your exposure and validate your claim:
